(You must complete the Lead Inspector Class before taking the Risk Assessor Class)
STEP 1. Complete the Student Information section: with the student information, est.
Student Name, Address, Phone # we can reach you at, and Email.
Company information can be added.
However, check the box to confirm
if the company address or student address will be used on the class certification.
(However, remember the information on this form is the student information)
STEP 2. Training Course:
Check the actual class the student will be taking;
also, put the date of the class.
(Remember a worker can take the Lead Abatement Contractor Class
and be license as a worker) But in this section, you must check the actual class you want to take.
STEP 3. Type of work interested in or currently performing for Development and or ODH Grant:
In this section check the type of work you are willing to do after completing the training,
(Remember that the counties you list can call and ask you to work in their county)
STEP 4. Select the Counties in which you are willing to participate:
(Remember when picking your counties this training is for rural areas (non-major metropolitan areas)
If you select State-wide, this means you are willing to work in any county that requests your services.
If you check the Statewide box you do not have to pick counties.
If you don't check the State-wide box,
you do have to check or mark the counties on the Ohio map
that you are willing to work in,
remember that the counties you list can call and
ask you to work in their county.
STEP 5. Student verification proof submitted for training:
In this section, you need to check if you are currently working with a program,
provide the program contact person information
and submit your proof, Ex: check, invoice, or something
with the program OR your company name.
If you are not currently working with a program
but willing to work with a program,
check the willingness to work with a program.
If there is a program that you have contacted
give the Program name, Contact Person, Phone #, and date of contact.
STEP 6. Student Signature:
You don't have to sign the form before you submit it, you can sign the form when you take the class!
The student must sign and print their name so that it can be recognized
as being the student that signed the form
remember this form has to be completed for each class the student wants to take.
STEP 7. Submission of Application:
Once the application is completely filled out, per the instructions listed above, you will save the appliaction as a PDF and send it to this email liannadctp@gmail.com as an attachment. In the subject bar you will put your first and last name.
Once the appliaction is received,
you will be notified, by email or by the phone number listed on your application.
(This application must be submitted at least five business days prior to the scheduled class you've selected)

Ohio Department of Health (ODH)
Workforce Development Licensure Fund Form
Individuals who completed the lead abatement contractor, lead abatement worker, Inspector and or Risk Assessor training may be eligible for funds to pay their intial license fees.
Funding is limited and available on a first-come, first-serve basis, or until funds are spent.
Interested individuals should carefully read all instructions and complete the
Workforce Development Licensure Fund Form at the end of the insturctions:
Submit a complete application to the Ohio Department of Health, Attn: Shannon Yiannaki, 246 North High Street, Columbus, Ohio 43215 .
Doing your application online will require you to pay for the license fees.

respond when a properly completed form is submitted.
Failure to complete the Student Lead Training Accreditation form listed after the instruction,
per the instruction will make you not eligible for the requested class.

After students show proof of working or will work with the below-listed program, they can take Lead Abatement Classes and their State Examination at no cost! To be eligible for this training opportunity, students must submit their proof of affiliation with one or more of the programs listed below before registering for a class. Students can take any course listed on LDCTP's Class Schedule. Don't wait, once funds are gone, this fantastic opportunity will end!
Once the student submits their eligibility proof, the student will then be able to signup for the requested class or classes at no cost to the eligible student. Also, the Ohio Department of Health will waive the ODH licensure fee for these individuals subsidized by this grant program.
Background
In Ohio, state agencies are collaborating to address the issue of lead exposure to Ohioans, especially children. ODH received funding in the biennial operating budget for lead hazard control training activities; however, there is a statewide shortage of lead abatement contractors who can perform this work. In partnership with ODH, Development will provide funding for lead workforce development and training activities to increase the number of lead contractors and workers who perform lead hazard control work statewide with an emphasis in rural areas.
Purpose
The purpose of the Training and Technical Assistance Grant Program for Lead Accreditation is to optimize the lead workforce in Ohio by offering frequent, accessible, and affordable training opportunities. Development's goal is to achieve training coverage statewide, free of cost to the participant, by reimbursing training providers for costs associated with offering these courses.
